How to build a customer form
A form is a questionnaire the customer fills in before the visit: a health declaration, treatment consent or preferences. Build it once and send it as a link.
Where to start
Navigation menu, Forms
Open the forms screen
Open the navigation menu and choose "Forms". This is where your forms and the answers you received live.
Create a new form
Tap the add button. You can start from a ready-made template and edit it, which is much faster than a blank page.
Add questions
Each question has a type: short or long text, number, date, single or multiple choice, yes or no, signature, or file upload. You can mark a question as required.
Add a sensitive-data notice
On a health form, write a sentence explaining why you collect the information and what happens to it. The customer sees it before filling in.
Decide how the form is sent
Auto-send to everyone booking services you choose, send manually to one customer, or turn on a permanent public link you can share anywhere.
Save and fill it in yourself
Save, open the preview and fill it in like a customer. That is the fastest way to catch a confusing question before anyone else sees it.
Tips from the field
Forms are available on Professional and above. How many active forms and monthly submissions you get depends on the plan.
A short form gets filled. Five questions that get answered beat fifteen that get abandoned.
Marking a form required does not block the booking. It highlights that the customer has not filled it in, so you can weigh that before approving.
Every submission stores the questions as the customer saw them, so editing later does not change older submissions.
Common questions
Inside the form, in the "Submitted" tab. You can search, filter by date, and open a submission to see everything including signature and files. A specific customer's submissions also appear on their customer card.
Yes. There is a signature question type, and the customer signs with a finger or mouse. The signature is stored with the exact questions they saw.
